Mileage Reimbursement Included.Position Summary:Provide patient care consultation and treatment to patients with serious illness with support from a collaborative physician.Essential Duties & Responsibilities:Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competenciesProvide direct client care consultations, to include participation in patient and family counseling and management of symptoms related to life-limiting conditions in collaboration with the interdisciplinary team of physicians, nurses, social workers, and chaplainsTake clinical call for palliative care clientsActively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of careCreate and maintain accurate patient recordsPractice within the scope of license, certification or registration, where applicable, and as required by law and regulationServe as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the communityMaintain current medical education and appropriate licensurePerform other duties as requested or assignedKnowledge, Skills & Abilities:Knowledge of Federal, State and Local regulations and Medicare Conditions of ParticipationKnowledge of HIPAA privacy rules and regulationsKnowledge of electronic medical records systemsBasic data entry skillsExcellent organization skillsExcellent interpersonal skillsExcellent written and oral communication skillsAbility to work under pressure to meet deadlinesAbility to take initiative and utilize innovative techniques and ingenuityAbility to interact with various departments throughout the organizationAbility to work flexible hours, if neededAbility to participate as a team player to coordinate projectsAbility to plan, prioritize and coordinate multiple projectsMinimum Qualifications:Education:Master's degree and clinical rotation, preferred.Experience:2 years of (post education) experience is requiredPalliative care experience preferredLicensures & Certifications: (if applicable)Current NP or PA Board Approved CertificationCurrent NC Medical License in good standingCurrent DEA Registration in good standingMeet and maintain eligibility requirements to be appointed medical staff and qualify for privileges at partner facilitiesValid driver's license and proof of automobile insurance.Summary of Physical Requirements:Walking, standing, sittingHand dexterity with gripping/grasping to include writingIntent & Function:All descriptions have been reviewed to ensure that only essential functions and basic duties have been included.
