Job Description:

RN Case Manager Job Summary

Deliver end-of-life care as an expert, developing trusting relationships with patients and their families. You will be valued and respected, with access to employee wellness and support programs, and incredible team support and empathetic leadership.

Assume full nursing responsibility for patient care, continuously evaluating performance and making necessary changes.
Makes the initial nursing evaluation within 48 hours of referral.
Identifies and re-assesses patient needs, initiating and coordinating the plan of care.
Documents patient progress and collaborates with the patient, family, and healthcare team.
Experience Requirements
- 1 year of clinical experience.
  License Requirements
  - Current RN licensure in the state of practice and 1 year of clinical experience.
  - Current CPR Certification.
  - Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
